The dog sold pursuant to the terms of this agreement is described as follows:
Microchip #: __________________________________________________
Name: Nestlewood _____________________________________________________________
Fourteen Day Health Guarantee
Nestlewood, LLC guarantees your new puppy is in good health at the time of delivery and has been examined by a licensed veterinarian. Nestlewood LLC assumes no liability for injury of the dog during or after transport to Buyer. We offer you a 14 day guarantee against disease.
The Buyer is asked to take the puppy to a licensed veterinarian for an examination within the first five (5) days of purchase. This gives you the assurance that your puppy is healthy and allows your vet to start your puppy on heart worm medication and begin a relationship with you early on in your pet’s life.
The Buyer agrees to hold the puppy in their arms or contain in a crate until you are provided with a sterilized examination room to reduce the risk of infection, in particular Parvo Virus. The puppy will not be taken to places other dogs have been, such as pet stores, dog parks, kennels, grooming salons, etc., as the puppy has not been fully vaccinated and Parvo virus lives in the soil.
If the puppy is found to have poor health by a licensed veterinarian within 14 days by New Hampshire law, upon immediate return and documentation by the examining veterinarian, the puppy will either be replaced with a puppy selected by the breeder from the next available litter, or purchase price will be immediately refunded. If Buyer chooses to keep the puppy diagnosed with poor health, Nestlewood LLC will reimburse for all reasonable veterinary costs, up to the cost of the puppy.
Two-Year Health Warranty
Nestlewood LLC provides a warranty up to the age of 2 (two) years to the Buyer, against genetic disease that severely impacts the quality of the dog’s life, provided that the Buyer has fulfilled the following requirements:
If a debilitating genetic disease that is severe, or life threatening is diagnosed by a veterinarian, it must be confirmed by a second veterinarian that is chosen by Nestlewood, LLC at our expense. The Buyer must supply any requested veterinary records, including diagnostic data, x-rays, laboratory testing, etc.. to Nestlewood LLC within ten days of diagnosis by a licensed veterinarian. If the puppy is euthanized or in any way disposed of, without a second opinion from a Nestlewood LLC approved veterinarian, the Buyer waives all rights to a replacement puppy or any financial payment as compensation.
This warranty does not include viral illnesses, infections, improper bites including base narrow canines, over bites or under bites, hernias, Giardia or Coccidosis or Parvo. It also does not include any illness due to the ingestion of foreign objects, food, chemicals or physical injury. This warranty does not pay for any dental procedures. This warranty does not cover allergies.
This warranty covers the following genetic diseases: Hip Dysplasia, Addison’s Disease, Cushing’s Disease, PRA, Von Willabrand Disease, and Epilepsy identified by the age of 2 (two) years. The Buyer has the option of either a puppy chosen by the Seller from the next available litter or reimbursement up to the purchase price of the puppy minus the $500 application processing fee, of all veterinary expenses related to the illness. Your puppy need not be returned to us. Shipping fees will not be refunded, and if it becomes necessary to ship a replacement puppy, those fees are to be paid by the Buyer.
If the puppy should die from a suspected genetic disease by the age of 2 (two) years, a licensed veterinarian acceptable to our veterinarian must perform a necropsy at the Buyer’s expense. If the necropsy shows a genetic disease as the reason for the death, then Nestlewood LLC will provide another puppy to the Buyer or refund up to puppy purchase price minus the $500 application processing fee. A written statement from the veterinarian must be provided, as well as a copy of all veterinary records. This warranty is void in the event of an indeterminable or inconclusive diagnosis.
The Buyer agrees to feed Life’s Abundance Dog Food for the first year of the dog’s life, which is not available in stores and must be purchased through your authorized pet representative, which is Nestlewood. After that they agree to either continue on the Life’s Abundance or feed their dog a premium quality food (4-5 star rating) as defined by Whole Dog Journal or Dog Food Adviser and give their dog regular exercise and maintain the dog at a healthy weight. All Life’s Abundance Dog food must be purchased through our Nestlewood account as Life’s Abundance keeps track of the receipts for us, which will be necessary until your dog’s first birthday, for your guarantee to remain in effect. Our account number is 20550385 and must be used when ordered by phone or online.
The Buyer will reasonably limit stair climbing and jumping the first year and postpone strenuous repetitive exercise for the first year, including jogging and agility training.
The Buyer agrees to provide active human companionship and socialization around other children and adults and animals so that the dog grows into a stable companion pet. The Buyer agrees to complete at least two (2) puppy/and or dog obedience classes by the age of one year (1) and provide written proof of completion by the training facility. If the Buyer does not complete two (2) obedience classes by one (1) year, this warranty is null and void.
The Buyer agrees to make and keep annual veterinary appointments for their dog and keep all immunizations current according to the guidelines of the American Veterinary Medical Association and properly administer heart worm preventative medication and flea and tick prevention. The use of Trifexis, and Comfortis for flea and tick prevention will render this warranty null and void.
The Buyer agrees that if the Buyer cannot take proper care of the dog, they will contact Nestlewood LLC and we will have the choice of either taking the dog back or assisting in finding a suitable home. At no time will the Buyer take the dog to a shelter or dog pound. In the event that re-homing the dog is not an emergency situation, the Buyer agrees to keep the dog until the Breeder can find a new home for the dog, so that it only needs to make one transition. Upon re-homing the dog, Breeder agrees to refund any proceeds minus any expenses that are incurred by the Breeder to the Buyer, for the dog during the re-home process. Expenses will be documented and include: training, boarding, veterinary care, grooming and care.
If a serious problem arises, we ask that you notify us immediately. You must provide veterinary records documenting the diagnoses of the genetic disease within 10 (ten) days of diagnosis. The microchip must be scanned and documented at this visit.
If the dog is diagnosed, as having genetic hip dysplasia the Buyer must submit x-rays to the Orthopedic Foundation for Animals (OFA) for a second opinion. OFA’s opinion of the hips will be the official opinion and a refund will only be issued if OFA determines the dog has hip dysplasia and if the dog’s veterinary records show that it has been maintained at his/her optimal weight since purchase. If the records show that the dog has a history of being overweight, the Buyer forfeits any monies back from the Seller.
If the dog is diagnosed with a genetic and debilitating eye disease that severely affects the dog’s quality of life, the dog’s eye exam must be sent to Canine Eye Registration Foundation (CERF) for judgement and the statement confirming a genetic debilitating eye disease by CERF, sent to the Seller.
We will have the option to have our veterinarian review the medical records and reports with the Buyer’s input so that we can determine together how to proceed. If our veterinarian agrees that the problem is serious and that this is not the result of injury or the dog being overweight, you may decide to keep the dog as your pet and in this event, Nestlewood, LLC will refund medical treatment up to the cost of the dog minus the $500 application processing fee. A refund of medical expenses requires proof of medical treatment from a licensed veterinarian. If the Buyer chooses not to provide medical treatment to the dog, there will be no refund of monies.
Adult size, conformation, coat quality, color and pattern of puppy/dog are not guaranteed. Nestlewood LLC assumes no responsibility for the puppy after it leaves the premises for medical expenses, mortality, puppy’s behavior, family disagreement, accidental injury or any other reason. It is understood that no warranty or representation has been made regarding this puppy except as set forth in writing in this agreement. It is further agreed that the puppy is of pet quality for the expressed purpose of a companion pet. This warranty shall apply to the original buyer and will be terminated upon transfer of ownership. In the event, Nestlewood Labradoodles stops breeding, no replacement puppies shall be given and this contract is null and void.
I have read and agree to the terms and conditions above.
